Ubisoft has bundled all their past few days giveaways into a single one to be redeemed all at once. Visit the giveaway page:

Register or log in to your account, then add the games to your Uplay library by clicking "PC". I believe the past in-game items will also be redeemed alongside the games
You only have until Dec 20th to claim the bundle, making it a 24 hour only giveaway.
